A sudden rise in blood creatinine levels after starting creatine supplements often causes concern among fitness enthusiasts, with many wondering whether they should stop using the supplement due to fears of kidney damage. However, nephrologists say that an increase in creatinine does not always indicate kidney failure, as creatine supplementation itself can naturally influence blood test results.
Creatinine is a waste product produced when the body breaks down creatine, a compound stored in muscles and used for energy production. According to Dr Bhanu Mishra, Nephrologist at Fortis Hospital, Shalimar Bagh, creatine supplements can lead to a mild rise in blood creatinine because additional creatine is converted into creatinine during normal metabolism. He said an increase in levels should be evaluated along with other kidney function tests rather than being judged through a single blood report.
Creatine is among the most researched sports supplements and is widely used by athletes, bodybuilders and fitness enthusiasts to improve strength, muscle growth and exercise performance. It helps replenish adenosine triphosphate (ATP), the body’s primary energy source during short bursts of intense activity. Studies have linked creatine use with improved workout performance, faster recovery and better results during repeated high-intensity exercises.

Dr Mishra said that most individuals can benefit from a daily maintenance dose of 3-5 grams of creatine, while the commonly used loading phase of around 20 grams daily for five to seven days is not essential. He advised users to take creatine with adequate water and preferably alongside meals containing carbohydrates and proteins, especially for those involved in regular exercise.
Experts caution that certain warning signs should not be ignored. A sudden or significant increase in creatinine levels, swelling in the feet or face, reduced urine output, severe dehydration, persistent fatigue, blood in urine or a history of kidney disease require medical evaluation. In such cases, stopping creatine temporarily and consulting a healthcare professional is recommended.
Creatine is generally considered safe for healthy adults when consumed at recommended doses, but people with chronic kidney disease, liver disease, pregnancy or breastfeeding-related concerns should avoid using it without medical advice. Doctors may recommend additional tests such as estimated glomerular filtration rate (eGFR), urine analysis or repeat blood investigations to determine whether a creatinine increase is harmless or linked to an actual kidney problem.
Medical experts suggest following safe supplementation practices, including using trusted products, maintaining proper hydration, avoiding unnecessary combinations of supplements and informing doctors about creatine use before undergoing blood tests. A higher creatinine reading alone does not confirm kidney damage, but continued elevation or accompanying symptoms should always be investigated to protect long-term health.
