Study Explains Why Indians Develop Type 2 Diabetes Earlier Than Western Populations

Type 2 diabetes, once regarded as a condition affecting middle-aged and older adults, is increasingly being diagnosed in Indians in their twenties and thirties, raising concerns among healthcare experts about a growing public health challenge. Doctors say India is witnessing the onset of diabetes nearly a decade earlier than many Western countries, driven by a combination of genetic predisposition, early-life nutritional factors and rapid lifestyle changes. With India also home to one of the world's youngest populations, the trend has significant implications for the country's healthcare system, workforce and economy.
According to health experts, India has one of the world's largest populations living with diabetes. Nearly one in seven Indian adults has diabetes, while almost one in four has prediabetes, placing millions at risk of developing the disease in the coming years. Many individuals remain unaware of their condition until complications affecting the heart, kidneys, eyes or nerves begin to appear. Early diagnosis remains a challenge, making routine screening and preventive healthcare increasingly important, particularly for younger adults with risk factors.
One of the primary reasons Indians develop type 2 diabetes at a younger age is a unique biological characteristic often referred to as the "thin-fat" Indian phenotype. Unlike many Western populations, Indians may appear lean but carry higher amounts of fat around the abdomen and liver while having comparatively lower muscle mass. This pattern increases insulin resistance, making it easier for blood sugar levels to rise even in people who are not overweight by conventional standards. As a result, experts caution that Body Mass Index (BMI) thresholds commonly used in Western countries may underestimate diabetes risk among Indians, underscoring the importance of assessing waist circumference and other metabolic indicators alongside BMI.

Doctors also point to the lasting impact of nutrition during pregnancy and early childhood. Poor maternal nutrition, low birth weight followed by rapid weight gain during childhood, and inadequate nutrition in the first years of life can permanently affect the body's ability to regulate glucose. These early-life factors may alter insulin production and metabolism, increasing the likelihood of developing type 2 diabetes later in life. This concept, often referred to as developmental programming, suggests that the foundations of diabetes risk may be laid even before birth, making maternal and child nutrition a critical component of long-term disease prevention.
Rapid urbanisation and changing lifestyles have further accelerated the problem. Sedentary work, reduced physical activity, increasing consumption of processed and calorie-dense foods, chronic stress and inadequate sleep all contribute to rising insulin resistance and obesity. While genetics increase susceptibility, experts emphasise that lifestyle choices often determine when the disease develops. Family history, smoking, excessive alcohol consumption and unmanaged stress can compound the risk, leading to an earlier onset of diabetes than seen in many Western populations.
Health specialists stress that the trend is preventable to a significant extent through early intervention. Regular health screenings, especially for individuals with a family history of diabetes, maintaining a balanced diet, engaging in routine physical activity, preserving healthy muscle mass and managing body weight can substantially reduce the risk of developing type 2 diabetes. They also advocate for improved maternal healthcare and better childhood nutrition to address the disease at its roots. With diabetes increasingly affecting Indians during their most productive years, experts say greater awareness, early detection and sustained lifestyle changes will be essential to slowing the country's growing diabetes burden.
