DGCA Mandates Random Drug and Alcohol Testing For Flight Crew

How India's aviation regulator tests pilots for drug use.

The recent Air India Phuket-Delhi flight incident has brought renewed attention to how pilots in India are tested for drugs and alcohol as part of aviation safety protocols. The pilot-in-command of the flight reportedly tested positive for marijuana in a confirmatory drug test following an incident in which the aircraft experienced a sudden altitude variation of around 300 feet during cruise. Initial findings by Airbus indicated that the aircraft, which was carrying 137 passengers, lost control for about four seconds. The incident remains under investigation, and no direct connection has been established between the pilot’s positive test and the aircraft’s in-flight problems.

Under regulations enforced by the Directorate General of Civil Aviation (DGCA), flight crew members are subject to testing for psychoactive substances. Airlines are required to randomly test a specified portion of their pilots each year, while additional tests can be conducted following accidents, incidents or when there is reasonable suspicion. Pilots are also required to undergo pre-flight alcohol testing through breathalyser checks. The purpose of these measures is to ensure that crew members are not operating aircraft under the influence of substances that could affect their judgment, coordination, reaction time or decision-making ability.

Drug screening generally begins with the collection of a urine sample under controlled conditions and strict chain-of-custody procedures. According to an airline first officer cited in the report, pilots are called for testing and provide their samples in designated facilities, with the process conducted in their presence. An initial screening can identify prohibited substances through a testing kit, while a non-negative result is sent to an accredited laboratory for confirmation. More advanced methods, including Gas Chromatography-Mass Spectrometry and Liquid Chromatography-Mass Spectrometry, can identify specific compounds with greater precision and help reduce the possibility of false positives. Testing can detect substances such as cannabis, cocaine, amphetamines, opioids and certain sedatives, as well as some medications that are restricted for pilots.

A positive drug test does not necessarily establish that a pilot was impaired at the precise moment of operating an aircraft because some substances and their metabolites can remain detectable in the body for days or longer after consumption. The science behind testing involves identifying either a drug or the chemical compounds produced when the body processes it. In the Phuket-Delhi case, preliminary assessments have also pointed towards a technical malfunction involving hydraulic systems, although the investigation is continuing. The incident nevertheless highlights the importance of India’s substance-testing regime, as aviation leaves little room for impairment and requires pilots to remain medically and mentally fit while operating aircraft.
