The move, set to phase out over two years, aims to eliminate duplication and redirect £200 billion in public funds to frontline services like nurses and doctors.

Starmer criticized the dual-layered system, questioning why taxpayers should fund overlapping roles—such as separate communications and strategy teams in NHS England and the Department of Health and Social Care (DHSC).

“I can’t explain to the British people why they should spend their money on two layers of bureaucracy,” he said. The overhaul will see NHS England’s 15,300 staff merged with the DHSC’s 3,300, with Health Secretary Wes Streeting targeting a 50% headcount reduction—potentially 10,000 job losses—saving hundreds of millions annually.

The Labour government frames this as a return to “democratic control,” promising a leaner NHS focused on cutting waiting lists. Starmer also touted AI and digital tools to streamline operations, projecting £45 billion in efficiencies.

However, critics warn of disruption. Former health officials argue the government already had oversight powers, while the Nuffield Trust cautioned that reorganizing risks diverting focus from patient care amid spiraling waiting lists and social care woes.

Streeting defended the move in Parliament, calling it the “final nail” in the coffin of the 2012 reforms, which he blamed for inefficiency. Starmer’s plan signals a seismic shift—whether it delivers remains a gamble watched closely by a nation reliant on its NHS.
