Surgeon Flags Four Exercise-Related Heart Warning Signs

Exercise is generally associated with better cardiovascular health, improved fitness and increased energy, but certain symptoms during physical activity should not automatically be dismissed as signs of ageing or being out of shape. Chest discomfort, unusual tiredness and lightheadedness can sometimes indicate an underlying health problem, making it important to recognise changes in how the body responds to exercise. A heart surgeon has highlighted four warning signs that people should take seriously when they occur during or shortly after physical activity. The symptoms can be easy to overlook, particularly when someone is returning to exercise after a long break or increasing the intensity of a workout.

However, a sudden or unusual change in exercise tolerance can warrant medical evaluation rather than simply being attributed to poor fitness. One of the key warning signs is chest tightness or discomfort during exercise. While strenuous activity can cause temporary muscle discomfort or breathlessness, pressure, squeezing or unusual tightness in the chest can have a different significance, particularly when it appears repeatedly with exertion. Heart-related symptoms can sometimes be subtle, so persistent or unexplained chest discomfort should not be ignored. Unusual fatigue is another symptom that deserves attention when it is disproportionate to the activity being performed.

Feeling tired after demanding exercise is normal, but a significant decline in exercise capacity or exhaustion during activities that were previously manageable can signal that something has changed. Such symptoms can have several possible causes, including cardiovascular problems, and should be assessed if they are persistent or unexplained. Lightheadedness or feeling faint during exercise is also a warning sign that should not simply be blamed on exertion. A temporary sensation can sometimes occur because of dehydration, overheating or other relatively minor factors, but recurrent or severe lightheadedness may require medical attention. Fainting or nearly fainting during physical activity is particularly concerning and warrants prompt medical evaluation.

Another important warning sign is unusual shortness of breath. Exercise naturally increases breathing, especially during vigorous activity, but breathlessness that seems excessive for the level of exertion or occurs unexpectedly can indicate that the heart or lungs are struggling to meet the body's demands. A noticeable change from a person's usual exercise tolerance is particularly important to discuss with a healthcare professional. These symptoms do not necessarily mean that a person has heart disease. Chest discomfort, fatigue, dizziness and breathlessness can result from numerous conditions, ranging from dehydration and respiratory problems to anaemia and other medical issues.

However, their occurrence during exercise makes it important to consider the possibility of a cardiovascular cause, especially in people with risk factors such as high blood pressure, diabetes, high cholesterol, smoking or a family history of heart disease. People who experience severe chest pain, significant breathing difficulty, fainting or symptoms that do not settle after stopping exercise should seek urgent medical care. Others experiencing recurring or unexplained changes in their response to exercise should speak with a healthcare professional before continuing strenuous workouts. The key message is not to assume that unusual symptoms are simply part of getting older or being unfit, particularly when they represent a new or noticeable change.
