Neurologist Explains Whether Artificial Sweeteners Actually Harm Your Brain Health

Artificial sweeteners, widely used as lower-calorie alternatives to sugar, have come under fresh scrutiny following a 2025 study published in the journal Neurology that linked higher consumption to faster cognitive decline. The research followed nearly 13,000 adults over eight years and found that those with the highest intake of low- and no-calorie sweeteners experienced more rapid declines in memory, verbal fluency and overall cognitive function, with the association appearing stronger among participants under the age of 60. Commonly examined sweeteners included aspartame, saccharin, acesulfame-K and certain sugar alcohols.
Hyderabad-based neurologist Dr Sudhir Kumar examined the findings and noted that the observed pattern raised legitimate questions about long-term brain health. He emphasised, however, that the study was observational in design and therefore demonstrated an association rather than definitive proof of causation. Individuals who consume larger quantities of artificial sweeteners may differ in other lifestyle or health factors that independently influence cognitive outcomes.
Dr Kumar further observed that earlier research has reported links between artificially sweetened beverages and an elevated risk of Alzheimer’s disease, though the overall body of evidence remains inconclusive. He cautioned against interpreting any single study as grounds for panic or for an immediate, complete elimination of these products from the diet. At the same time, he advised against treating artificial sweeteners as an unrestricted healthy substitute for sugar.

The neurologist recommended a more measured long-term approach centred on reducing overall preference for sweetness rather than simply swapping one sweetening agent for another. Practical steps include choosing water in place of sweetened drinks, selecting fruit instead of desserts, and gradually adjusting the palate to accept foods with less added sweetness. Such changes, he suggested, address the underlying habit of seeking intense sweet tastes.
FDA-approved artificial sweeteners such as saccharin, acesulfame, aspartame, neotame and sucralose continue to be widely available and are generally regarded as safe within established intake limits. The recent findings do not overturn that regulatory position but do highlight the value of moderation and of prioritising whole-food dietary patterns.
