Dark Circles Under One Eye: Surgeon Explains Causes, Shades And Medical Attention
Expert explains causes and warning signs of one-eye dark circles.
Dark circles under the eyes are often considered a cosmetic issue linked to lack of sleep or fatigue, but experts warn that the condition can sometimes indicate underlying health concerns, especially when it appears under only one eye or is accompanied by other symptoms. Dr A.A.V. Ramalinga Reddy, senior cataract surgeon at Dr Ramalinga Reddy Maxivision Super Speciality Eye Hospitals, explained that under-eye darkness can develop due to multiple reasons and cannot always be addressed through skincare products alone.
According to the eye specialist, dark circles may occur due to skin pigmentation, thinning of the skin beneath the eyes, visible blood vessels, loose lower-eyelid skin, under-eye bags or prolonged eye irritation. Temporary factors such as stress, inadequate sleep and exhaustion can also contribute to their appearance. Dr Reddy cautioned that because the causes vary from person to person, a single cream or cosmetic treatment may not be effective in reducing dark circles.
The colour of under-eye darkness can also provide clues about its possible cause. Brown-coloured circles are often associated with pigmentation, while blue or purple shades may appear when blood vessels become more visible through the thin skin around the eyes. Changes in darkness depending on lighting conditions may indicate structural shadows caused by facial features, while puffiness along with itching or congestion could point towards allergies.

The specialist explained that dark circles are frequently caused by a combination of factors rather than a single issue. This is why identifying the underlying reason is important before choosing any treatment approach. While many cases are harmless and related to lifestyle factors, certain changes in appearance or accompanying symptoms should not be ignored.
Dr Reddy advised people to seek medical attention if darkness or swelling develops under only one eye, becomes progressively worse, or continues along with noticeable skin problems or allergy-related symptoms. He also recommended prompt evaluation in cases involving pain, redness, changes in vision or unusual discolouration after an injury, as these signs may require medical assessment.
The doctor added that most cases of dark circles do not indicate a serious health problem and can be managed by addressing the underlying cause. However, sudden changes, one-sided symptoms or additional warning signs should be evaluated by a healthcare professional rather than being treated only as a cosmetic concern. Dr A.A.V. Ramalinga Reddy has more than 35 years of experience in eye care.
